Anesthesia/Surgery Consent Form - Necessary for All Services Requiring Sedation/Anesthesia

We take the utmost care in providing your pet with a safe and comfortable anesthetic or sedation procedure, including careful attention by our trained staff, electronic vital signs monitoring and IV fluid therapy, but there is risk associated with anesthesia. Please closely review the form below, fill out and sign the acknowledgment and bring with you when your pet is admitted for such a procedure. This form is available in our hospital, if you choose to fill out at the time of admittance. Please keep in mind, we recommend your pet is admitted with an empty stomach, which means no food or water after mid-night prior to the day scheduled. Our staff will contact you when the procedure is completed to let you know the status of your pet in recovery. Anesthetic procedures are typically performed mid-day, so updates are generally late afternoon. However, you are welcome to contact us at anytime to inquire about your pet or ask questions. Some surgeries require an overnight stay, however if your pet is undergoing a outpatient procedure, you are welcome to pick them up that day before we close at 6:00pm. Please consult with our staff as to whether your pet's procedure is scheduled for outpatient or requires an overnight stay.

Euthanasia Consent Form - Required Prior to Service

We understand the difficulties of electing to euthanize your pet. The form below is required prior to euthanasia so in an effort to ease this process, you may elect to download the form below, fill it out in advance and bring with you to the hospital rather than filling it out in the hospital. This form is available in the hospital however, if you choose to complete it then.
